2024-2025 Reimbursement Form / Expense Voucher for LSPO

Please find below the appropriate form to use for reiumbursement related to running LSPO events, activities, and/or materials required in order to do so. Please note that we will need a copy of your receipts in order to issue a reimbursement.

How to use this Form:

1) Click on the link below to open the form

2) Download as a PDF or Word/Google doc, onto your own computer

3) Fill out the form with your details, especially the reason for the reimbursement (such as what event or activity it related to)

4) Email your completed form AND RECEIPT(S) to treasurer@lspo.org

The Lincoln-Sudbury Parent Organization is a non-profit organization made up of the parents and guardians of Lincoln-Sudbury Regional High School students. The LSPO is dedicated to facilitating engagement and communication between families and the school, as well as partnering with faculty and administration to support underfunded staff needs, student activities and efforts that encourage school spirit.
